Please take a moment to review the samples you have selected and fill in your desired quantity and preferred color for each.

If you would like to receive the same item in multiple colors, please select “Various” from the Standard Colors drop-down menu, then state which colors you would like to receive in the Comments box on the next page. If “Various” is not listed, the item is only available in the color listed.

We request you limit your order to 24 pieces total. If your order exceeds 24 pieces or if you have placed more than one order in a day or more than three orders in two weeks, you must provide a shipper number on the next page or your order will be delayed.

Any sample orders received after 1:00 CST may not be shipped until the following day.
